Things to Do to be Ready

for the Flu Clinic
1. Download and print the Consent Form

2. Fill out Form for each person to get a shot.

3.   Bring the form to Worship on October 2       or mail it to:
          Condit Flu Clinic
          Condit Presbyterian Church
          11502 Hartford Road
           Sunbury, Ohio 43074

4. Bring your Insurance Card to the Clinic on October 9th if you did not complete forms

5. If you are not covered by insurance the
shots will be $22
